Independence Assessment & Personalised Plan

 

What’s included:

Home visit lasting approximately 90–180 minutes

Holistic assessment covering issues around health, mobility, routines, risks, time pressures, logistical problems and day-to-day pressures

Identification of key challenges and pressure points 

A written, personalised Independence Plan

Clear, practical recommendations, including services, people, and changes that may help

Follow-up phone call to talk through the plan and next steps

Price:

Ongoing Coordination & Support Packages

Once your Independence Plan is in place, you may choose ongoing support to help coordinate services, adjust plans as life changes, and provide reassurance for you and your family.

You can move between support levels at any time as your needs evolve.

 

Light Support

For people who are mostly managing but value reassurance and occasional guidance.

Includes:

Named point of contact

Occasional check-ins and wellbeing reviews

Minor plan adjustments as needed

Advice and signposting to appropriate services

£75–£120 per month

 

 

 

Active Support

For people who want regular support coordinating services and reducing day-to-day stress.

Includes:

Single, named point of contact

Regular check-ins

Active coordination of services and support

Liaison with professionals and providers where appropriate

Ongoing review and adjustment of your plan

Optional updates and reassurance for family members

Complex Support

For people with higher, changing, or more complex needs.

Includes:

Priority access to your coordinator

Frequent contact and proactive oversight

Coordination across multiple services and professionals

Support in higher-risk or fast-changing situations

Focus on stability, reassurance, and crisis prevention
